1. Bring Necessary Documents: Ensure you bring a valid ID and your insurance card to your appointment. If you do not have insurance, we offer private payment options starting at $75 per consultation.
2. Review Insurance Policies: We accept Blue Cross Blue Shield, Medicare
Traditional, Medicaid, Molina, United Healthcare, Superior, and Imagine 360. If you have any questions about your coverage, please contact our office.
